Assessment
- Assess the patient’s perception of their body, including their size, shape, and appearance.
- Identify any factors that may have contributed to the patient’s altered body image, such as weight loss, surgery, or a traumatic event.
- Assess the patient’s coping mechanisms for dealing with their altered body image.

Goals
- The patient will express a more positive body image.
- The patient will develop healthy coping mechanisms for dealing with their altered body image.
Interventions
- Provide the patient with accurate information about their body and its changes.
- Encourage the patient to talk about their feelings about their body.
- Help the patient to identify their strengths and positive qualities.
- Teach the patient healthy coping mechanisms for dealing with their altered body image, such as exercise, yoga, or meditation.
- Refer the patient to a therapist or counselor if needed.
Evaluation
- Evaluate the patient’s progress towards their goals at regular intervals.
- Make adjustments to the care plan as needed.
